Since 2010, over 400 people have undergone surgery under local anaesthesia by Dr Vanni Veronesi to treat occult tethered cord syndrome. “The diagnosis is complicated, and they are often sent to a psychiatrist”.

 

There are approximately 400 Italian patients from 17 Italian regions but also from abroad, including twenty-nine European patients, three patients from the United States, two patients from Brazil, one patient from Africa, and in the coming months the first patient resident in Russia will come to Italy - to undergo surgery at the “degli Infermi” hospital in Faenza by neurosurgeon Vanni Veronesi, of the Simple Departmental Structure of Peripheral Nervous System Surgery of the Department of Neurosciences of the AUSL of Romagna.

His innovative method represents a sort of unicum for treating tethered cord syndrome and occult tethered cord syndrome. These rare diseases are almost unknown in the medical community and difficult to diagnose.

Dr Veronesi also deals with this by publishing articles in authoritative international scientific journals and using particular instrumental support, such as lumbar magnetic resonance imaging in the prone position. However, the diagnosis is still clinical based on the symptoms and the outcome of the medical examination, including specific tests.

«My patients are operated with an innovative minimally invasive surgical technique under local anaesthesia, which I have been performing since 2010 - says Veronesi himself - which reduces the possible complications of traditional surgery to just cutting the skin, which in minimally invasive surgery is a few centimetres, while it does not present the various complications, sometimes clinically relevant, of classic surgery under general anaesthesia».

Often, patients with occult tethered cord syndrome are adolescents or young adults who have symptoms in the lower limbs, low back pain and difficulty controlling their sphincters - especially urinary and sometimes faecal sphincters - and a normal lumbar MRI.

"For this reason - confirms Dr Veronesi - patients often do not have the correct diagnosis, and the most frequent thing that happens is that they are sent to see a psychiatrist because it is assumed that there are psychological problems, adding a problem to the problem".

Obtaining a diagnosis of these syndromes is not easy, so even today, patients—Italian and foreign—often share their problems and experiences on social media. " They end up finding information on my website and then contact me," explains the neurosurgeon from Cesena.

It is a procedure - under local anaesthesia - that lasts about 25 minutes. "After an hour, the patient can already walk again, and discharge from the hospital generally occurs on the same day".

Free surgery is available for Italian patients—covered by the national health service—but not for foreigners residing in Europe. In most cases, the European directive for cross-border care is not respected, and the cost can exceed 11 thousand euros.

"The results are sensational - concludes the doctor - we collected testimonies of people who had walking disorders, filming them before and after our intervention and thus managing to document the incredible and immediate benefits".

Depuis 2010, plus de 400 personnes ont été opérées sous anesthésie locale par le Dr Vanni Veronesi pour traiter le syndrome de la moelle attachée occulte. « Le diagnostic est compliqué et les patients sont souvent adressés à un psychiatre ».

 

Il s'agit d'environ 400 patients italiens provenant de 17 régions italiennes mais aussi de l'étranger, dont vingt-neuf patients européens, trois patients des États-Unis, deux patients du Brésil, un patient d'Afrique, et dans les prochains mois, le premier patient résidant en Russie viendra en Italie - pour être opéré à l'hôpital « degli Infermi » de Faenza par le neurochirurgien Vanni Veronesi, de la Structure Départementale Simple de Chirurgie du Système Nerveux Périphérique du Département de Neurosciences de l'AUSL de Romagne.

Sa méthode innovante représente une sorte d'unicum pour traiter le syndrome de la moelle attachée et le syndrome de la moelle attachée occulte. Ces maladies rares sont presque inconnues dans la communauté médicale et difficiles à diagnostiquer.

Le Dr Veronesi s'en occupe également en publiant des articles dans des revues scientifiques internationales faisant autorité et en utilisant un support instrumental particulier, comme l'imagerie par résonance magnétique lombaire en position ventrale. Cependant, le diagnostic reste clinique, basé sur les symptômes et le résultat de l'examen médical, y compris des tests spécifiques.

«Mes patients sont opérés avec une technique chirurgicale mini-invasive innovante sous anesthésie locale, que je pratique depuis 2010 - dit Veronesi lui-même - qui réduit les complications possibles de la chirurgie traditionnelle à une simple incision de la peau, qui en chirurgie mini-invasive est de quelques centimètres, alors qu'elle ne présente pas les diverses complications, parfois cliniquement pertinentes, de la chirurgie classique sous anesthésie générale».

Souvent, les patients atteints du syndrome de la moelle attachée occulte sont des adolescents ou des jeunes adultes qui présentent des symptômes au niveau des membres inférieurs, des lombalgies et des difficultés à contrôler leurs sphincters - en particulier urinaires et parfois fécaux - et une IRM lombaire normale.

"Pour cette raison - confirme le Dr Veronesi - les patients n'ont souvent pas le bon diagnostic, et le plus souvent ils sont envoyés chez un psychiatre parce qu'on suppose qu'il y a des problèmes psychologiques, ajoutant un problème au problème".

Obtenir un diagnostic de ces syndromes n'est pas facile, c'est pourquoi, même aujourd'hui, les patients - italiens et étrangers - partagent souvent leurs problèmes et leurs expériences sur les réseaux sociaux. "Ils finissent par trouver des informations sur mon site et me contactent ensuite", explique le neurochirurgien de Cesena.

Il s'agit d'une intervention - sous anesthésie locale - qui dure environ 25 minutes. "Au bout d'une heure, le patient peut déjà remarcher, et la sortie de l'hôpital a généralement lieu le même jour".

L'intervention est gratuite pour les patients italiens - couverts par le service de santé national - mais pas pour les étrangers résidant en Europe. Dans la plupart des cas, la directive européenne sur les soins transfrontaliers n'est pas respectée et le coût peut dépasser 11 000 euros.

« Les résultats sont sensationnels - conclut le médecin - nous avons recueilli des témoignages de personnes qui souffraient de troubles de la marche, en les filmant avant et après notre intervention et en parvenant ainsi à documenter les bénéfices incroyables et immédiats ».
