FILUM TERMINALE SECTIONING WITH MINI-INVASIVE SURGERY: OPERATIVE MOVIE - Vanni VERONESI MD

In the surgical video the Brazilian patient is awake during the surgery and speaks with the interpreter Prof. Giorgio Charissiadis (Adjunct Professor at the University of Macerata) who describes the surgical steps. The minimally invasive surgical technique is performed under local anesthesia and involves an incision of the skin of a couple of centimeters, as can be seen in the monitor of the surgical microscope. The surgical microscope is used in the central part of the surgery in which the microsurgical technique is applied for the section of the filum terminale.

OVERVIEW VIDEO: interview on the "Buongiorno Regione Emilia Romagna" program of RAI3 on November 21st 2017 on the scientific publication concerning the minimally invasive technique of extradural filum terminal section under local anesthesia.

 

The microsurgical technique for minimally invasive surgery, under local anesthesia, of Extradural Filum Terminale sectioning is used by us since 2010 and was published, for the first time in the world, in a scientific journal in 2018, precisely in the American magazine Operative Neurosurgery.


After the publication, in 2013, of the scientific work of the Japanese colleagues of the University of Hiroshima on lumbar MRI in a prone position for the instrumental support for the clinical diagnosis of the occult tethered cord, we adopted, from 2014, a cutting-edge protocol with the aforementioned lumbar MRI in a prone position and with our minimally invasive surgical technique.

After years of searching for the cause of my problems, there was a suspicion that I suffered from a rare condition called occult pith syndrome. The doctors in my country know very little about this condition, so we needed help to get the right diagnosis. On the internet, I found a Dr. Veronesi e-mail address and asked him if he was also dealing with international patients. A few days later, the answer was in my mailbox: of course I treat them, what's your problem, what needs to be solved?
First he helped me get a correct diagnosis, he sent me a document describing a way to do a prone MRI, thus allowing me to do lumbar MRI in a prone position at the hospital in my country. He had no obligation to do so, I live in another country. I knew immediately that he is an extraordinary man. Then I went to Italy for the first time, and the surgery was scheduled for about a year later. The intervention is quick and painless and the hospital stay is short. My recovery was incredible, my condition improved very quickly after surgery: there was no back pain, the pain in my legs was gone, the urological condition started to improve and I was sleeping much better than before of surgery. One year after surgery my condition has improved further, only minor urological problems are still present. It is important to note that I am 38 years old, so I am a case of an occult adult bone marrow syndrome, neglected for years because the doctors in my country didn't know how to help me. I didn't know what to expect, but surgery was effective and successful. Now my life is easier, my problems are minor after surgery. This is the professionalism of Dr. Veronesi, he is truly remarkable, honest man, not easy to find these days. Thanks also to Dr. Sacco and all the hospital staff. Italy should be proud of you!
